A Looming Health Crisis: The Impact of Subsidy Lapse on Millions of Americans
In a worrying development, the recent expiration of enhanced premium subsidies under the Affordable Care Act (ACA) is poised to have a significant impact on health insurance coverage for millions of Americans. With soaring health premiums, the future of healthcare access is uncertain, and the political landscape is set to be influenced by this critical issue.
According to data released by the Centers for Medicare & Medicaid Services (CMS), approximately 22.8 million Americans have signed up for 2026 health insurance via the ACA marketplace, a decline of about 1.5 million from the previous year. This early sign of fallout from the subsidy lapse has health experts concerned about the potential loss of coverage for millions.
The enhanced ACA subsidies, which made insurance more affordable, played a crucial role in reducing the number of uninsured Americans. Total ACA enrollment doubled to over 24 million by 2025, but without these subsidies, the average recipient will face a substantial increase in premiums, with some estimates suggesting a more than doubling of costs.
"The expiration of the enhanced subsidies has reopened the coverage debate," said Jonathan Burks, executive vice president for economic and health policy at the Bipartisan Policy Center. With coverage becoming a greater focus, the question arises: How many will drop their insurance altogether?
Nearly 5 million people may drop their ACA coverage, according to health experts. The CMS data, which doesn't yet cover the entire open-enrollment period, suggests that more households are expected to opt out in the coming months. As people start paying their initial premiums, the true impact of the subsidy lapse will become clearer.
"It's too soon to say how big of a drop this will be," said Jared Ortaliza, a policy analyst at KFF. "New data shows fewer people are signed up for ACA plans compared to last year. If this trend continues, it would mark the first decline in year-to-year signups since 2020."
The Urban Institute estimates that 4.8 million people will lose their health coverage and become uninsured in 2026 due to lapsing subsidies. This highlights the financial risk of forgoing insurance, especially given the overall increases in healthcare prices.
"The risk of having an unmanageable health bill is real," Burks emphasized.
Additionally, another 2.5 million people are expected to drop their ACA coverage but find alternative insurance options. Some may turn to employer-sponsored coverage or Medicaid, depending on their circumstances. Others may opt for high-deductible plans within the ACA market, which offer cheaper premiums but higher upfront costs for care.
"A healthy and productive labor force is essential for a thriving economy," said Jessica Banthin, a senior fellow at the Urban Institute. "If large segments of the working-age population can't access healthcare, it ultimately harms the economy."
As the congressional midterm elections approach, the expired subsidies are shaping up to be a key political issue. Overall ACA enrollment has more than doubled since 2020, with most of the growth occurring in states won by President Donald Trump in the 2024 election. This has led to a partisan divide, with Democrats pushing for an extension of the enhanced subsidies and Republicans largely opposed to the idea.
A group of 17 House Republicans voted with Democrats to extend the subsidies, but the measure faces an uphill battle in the GOP-controlled Senate. President Trump has also threatened to veto such a measure.
